Vision
CHD will collaborate with our communities and other appropriate healthcare entities to become the driving force for:
(A) The provision of quality healthcare services outside the traditional hospital setting and
(B) The provision of appropriate healthcare education and training.
CHD will commit to :
(C) Work with the community to identify healthcare needs,
(D) Develop a strategic plan, and
(E) Serve as a community-based grant-maker.
